How to Apply
All applications will be evaluated by the Selection Committee (the “Committee”), which is composed of four members of different professions. After receiving approval from the Committee, the applicant would be recommended to the Foundation for a discretionary grant according to the Committee.
Individual applicants:
An individual applicant shall submit the following documents:
Application Form of the Vita Green Charitable Foundation
"For Application Class 1"
"For Application Class 1"
Copy of the valid Hong Kong ID card; Current address and telephone number; Income certificate; Certificate of health of the applicant
Recommended / expected and Expected deadline of receiving the Vita Green health product;
Copy of the valid Hong Kong practicing certificate of the enquirer (Definition of enquirer: registered medical practitioner, non-profit organisation or social welfare organisation)
Current address and telephone number of the enquirer; Registration number of the valid license of the enquirer and name of his/her association/ organisation

* After submission of the application, the Selection Committee will send a letter of confirmation to acknowledge receipt of a full application form and the above documents.
* If the application submitted requires urgent attention, the applicant may notify the Selection Committee in writing upon submission of the above documents. Otherwise, the applicant should expect to receive a letter notifying the result from the Foundation within three months after submission. Upon successful application, the applicant shall be notified on the details of the grant.
* All materials submitted by the applicant and the enquirer will be kept strictly confidential and will not be disclosed to any third party.
* The Foundation reserves the right and has full discretion to reject or not select any nomination or submission for any reason.
Organisational applicants
Any professional, non-profit organisation or social welfare organisation registered in Hong Kong may submit its application with the following documents directly to the Foundation.
The applicant shall submit the following to the Foundation:
Application Form of the Vita Green Charitable Foundation
"For Application Class 2"
"For Application Class 2"
Outline and details of the articles of association
Annual report and Audited financial statements of last period
Valid certificate of incorporation of non-profit or charitable organisation
Registration number of the valid license, and current address and telephone number of the organisation
Resumes of the personnel responsible for the organisation
Proposal use of funds and an evaluation of the project’s feasibility
Expected health product and/or desired financial grant
Other funding sources (specify background information if any)
